So, I can see what it is and how it is attached to a sword, but how does it benefit me? Does it keep the sword more secure? Also, does the koshiate face inward or outward?

I think the Idea was to get the katana out of the way so you could fight with out getting tied up on the hilt. If you have ever tried to fight with a katana in your armor sash it is very binding.
It could also be useful if you needed to mount and didn't have a tachi. It's hard to wear a sword in you sash and draw on horseback.
I made a pair for my retainer and I for the Japanese battle at Pennsic a few years ago. They worked well and were a convenient way to get all the weapons up to get inspected(and they look cool).
Just my thoughts. I'm not sure if I'm right though.

Thank you Torashi- I am still not clear on how the Koshiate gets the katana out of the way...? Does the koshiate hang off the sash by cords, and the sword is thrust through the koshiate straps?

correct the sword is strapped to the koshiate . the koshiate is tied on around your obi and allows you to move easier instead of having your katana pushed under your obi and thru your hakama . you can also tie it snug instead of tight so you can rotate it to your lower back for easier naganata or yari use.
